
This Advanced Practice Clinician provides acute care to clinic patients on an extended hours basis (regularly to include early mornings, evenings and weekend hours). The APP will work with a team of dedicated providers to ensure such patients receive the care and support that they need.
The job is a non-paneled position working at Legacy Medical Group: Camas located at 1625 SE 192nd Ave Ste 100, Camas, WA. This position supports Legacy Medical Group Primary Care providers and their respective patient panels.
This is a fully in-person position available to qualified clinicians licensed in the states of WA and OR.
QUALIFICATIONS:
Education:
As required by licensure.
Experience:
- Minimum one year of clinical experience within an urgent and/or primary care setting, required.
- Greater than four years’ clinical experience in family medicine, strongly preferred.
Certification/Licensure:
- National certification as an APC (NP or PA) required.
- Certification required in both Oregon and Washington (NP or PA).
- DEA required.
GENERAL ACCOUNTABILITIES and ESSENTIAL FUNCTIONS:
- Provides in-person acute care (virtual, when appropriate), emphasizing accessibility, health promotion, illness prevention and continuity of care to patients presenting with both minor and complex health issues.
- Regular hours to include early morning, evening and weekend hours to accommodate patient needs.
- Performs minor procedures as appropriate in an acute care setting.
- Appropriately triages patients to the proper site of care and works with support staff to schedule patient for appropriate follow-up care, as needed.
- Takes and documents a thorough history, exam, and assessment/ plan.
- Manages common acute and chronic illnesses.
- Discusses health concerns/diagnosis with patients, and families, collaborating with the Primary Care Provider (PCP). Together, PCP and APC determine an appropriate plan of care.
- Follows, develops, and reviews clinical protocols and best practice guidelines with leadership.
- Escalates patient care to higher levels of care as clinically appropriate.
Quality of Service:
- Thoroughly documents all patient encounters in a timely manner.
- Adheres to clinical practice guidelines as adopted by Legacy Medical Group.
- Complies with medical records policies regarding complete and accurate documentation of patient care. Initiates and maintains required records and legal documents.
- Participates in quality improvement activities.
- Counsels patients, families and significant others regarding one’s health, illness(es), while promoting health maintenance.
- Maintains a professional work environment and appearance in compliance with Legacy’s professional appearance policies.
- Participates in on-going education to maintain clinical knowledge and skills.
Teamwork:
- Works collaboratively with multidisciplinary team to provide treatment, ongoing follow-up and evaluation of the plan of care with patient.
- Works with and communicates with other providers in the clinic, including the medical director.
- Collaborates with other providers and medical directors within Legacy Medical Group.
- Demonstrates constructive problem solving with colleagues and clinic staff.
- Supports established policies and procedures of the clinic. Makes recommendations and contributes to policy formulation.
- Arrives on time, present and ready for duties at scheduled start time.
- Completes required documentation for referrals and consultations for prior authorizations.
- Returns patient telephone calls, responds to message within Epic in-basket, reviews result and completes refills related to acute care visits.
- Orders, interprets, and evaluates diagnostic tests to identify and assess the patient's clinical problems and health care needs as medically necessary.
- Participates in teaching and mentor of colleagues as needed.
- May be asked to assist other clinics with acute care visits.
Administrative:
- Contributes to and participates in department meetings.
Miscellaneous:
- Demonstrates excellent customer service skills.
- Understands and is able to effectively communicate HIPAA compliance, corporate compliance and client confidentiality.
- Ensures and/or remains in compliance with local, state, and federal regulations and accrediting standards.
