    The Behavioral Health Clinician (BHC) works in a consultative and collaborative role as an integral member of the primary care team. They provide behavioral health interventions to primary care patients with the goal of improving overall health of patients.   The BHC helps address a broad spectrum of behavioral health needs in the primary care patient population. Their goal is to aid in early identification and provide problem focused interventions that promote wellness and relieve symptoms. Behavioral issues that are addressed may be related to physical or mental  health conditions. They provide assessment, management and triage of psychological and social problems in the clinic patient population.   This position requires advanced level skill in delivering evidence-based interventions targeted to improve overall health for patients with a variety of emotional and behavioral problems. The work includes the BHC meeting with patients individually for an assessment and then providing recommendations to the primary care team to enhance the patient's care plan.  When indicated, the BHC may also work with patients for several subsequent sessions to provide on-going skills training and coaching to improve health and functioning. The BHC also supports helping patients, who require services not provided in the LMG clinic, to make a smooth transition to care with community partners.   QUALIFICATIONS:   Education:   Must be a graduate of a doctor of psychology program accredited through the American Psychological Association (APA) or have a Master of Social Work degree from an accredited school of social work.   Licensure: Must hold a current valid Oregon License as a Psychologist (PsyD or PhD) or Clinical Social Worker (LCSW)   Experience: Minimum of two years of clinical experience or healthcare related field.   Skills: - Ability to screen for common mental health problems. Able to conduct Cognitive-Behavioral or solution focused interventions. - Recognizes and advocates for patients regarding behavioral health strategies/interventions. - Promotes health behavior change through evaluation, brief intervention and education. - Works with multidisciplinary team to help support the patient’s health. Coordinates with LMG providers and community partners to refer for appropriate mental health therapy. - Demonstrates competencies in assessment of emotional, cognitive and behavioral disorders in child, adolescent, adult and/or geriatric patients (depending upon population served by specific position).  - Demonstrates competency in assessment of cognitive and psychological problems associated with, and/or exacerbating medical conditions. - Experience with working with an interdisciplinary team. - Skilled in case conceptualization and consultation. The NNP role has opportunities at both Level III and IV NICU units within Legacy.  Both units offer delivery attendance, medical management of high-risk neonates and infants, including rounding on NNP service patients.  Telemedicine Resuscitation coverage of other Legacy level I nursery sites is also a vital part of the NNP role.    The Level IV 45 bed unit is within Randall Children’s Hospital, a free-standing 9 story children’s hospital, and offers comprehensive care with all subspecialties, including cardiac and neurology surgery specialties, genetics and ECMO, within the framework of family integrated care.  Randall Children’s Hospital has a dedicated children’s transport team.  No transport requirement for the NNP role.  Rotating 12-hour shifts.   The Level III 22 bed unit at Legacy Salmon Creek Medical center is eagerly awaiting the addition of NNP’s to join the care team.   The schedule is emerging and includes 12, 16 and 24-hour shift options.    The role requires dual licensure in Oregon and Washington. Rotation frequency through both units is individualized to maintain 24-hour coverage at both sites, including nights, holidays, and weekends.  Holiday tracks and a well-established self-scheduling model bring increased satisfaction and balance to the team. QUALIFICATIONS AND ESSENTIAL FUNCTIONS: Patient Care Management:  The NNP’s work collaboratively with Neonatologists and pediatric subspecialists in the NICU as well as pediatricians and obstetricians in the Labor and Delivery and Family Birthing centers to guide the care of neonates and infants across the care continuum.  The NNP’s are able to practice in an autonomous role utilizing all technical procedural skills as well as essential decision making in the care of these patients.  Nursing Care Support:  The NNP team provide ongoing support to the nursing staff whether on the unit or through formal education opportunities.  The NNP’s are key leaders in patient care projects that support both nursing care and medical management.     In collaboration with the multidisciplinary treatment team, the Certified Drug and Alcohol Counselor provides trauma-informed drug and alcohol assessment, evaluation, brief intervention and referral to treatment for patients with substance use disorders (SUD) or co-occurring disorders (concurrent mental health and substance use disorders).     Provides formal and informal skills training, education and consultation for staff and members of the patient care team.  Direct supervision is provided by department leadership who will assess performance based on personal observation of work performed, chart review, feedback from department staff, patients and other relevant sources.   Education: As required by licensure/certification.  Masters degree in Behavioral Science field preferred.   Experience: Minimum of two year’s clinical experience working in an adult drug and alcohol treatment setting. Additional experience working in a mental health setting preferred. Demonstrated training and competence in motivational interviewing, culturally specific assessment, trauma-informed care in a substance use disorder or co-occurring disorders treatment setting. Experience with CBT and DBT preferred. Demonstrated experience in the development and monitoring of plans that are designed to meet patient’s individualized needs.   Licensure/Certification:  CADC I required, within 6 months of hire. For emergency coverage the group of Hospitalists are credentialed at multiple sites.    As an OB/GYN Hospitalist at LMH you will be part of Legacy Medical Group (LMG) Women’s Health Clinic. The group practices collaboratively, with outpatient CNM and OB/GYN clinics as well as 24//7 in-house Certified Nurse Midwifery presence on Family Birth.  OB/Gyn generalists are on campus and in clinic during the day, with transition to OB Hospitalist in house coverage at night and on weekends. The group works closely with Legacy Medical Group Maternal Fetal Medicine physicians, who have a clinic presence during the week and collaborate on pregnant people requiring transfer to Randall Children’s for a higher level of care.   The OB/GYN hospitalist also provides coverage for the Emergency Department and inpatient gynecologic consults and emergencies (miscarriage, ovarian torsion, ectopic pregnancy, h backup available.  Dedicated in-house Anesthesia is present 24/7.   Shifts are either 12 hours during the week (7p-7a) and either 12 or 24 hours  on weekends.  These shifts are in-house where there is a  well-appointed call room on the Labor & Delivery unit at Legacy Mount Hood.  The hospital boasts a history of excellent collegiality among all departments and a staff committed to each other and the community.
